The Science

Why 4 Implants Can
Support a Full Arch

The All-on-4 concept is based on a biomechanical insight: by tilting the two posterior (back) implants up to 45°, they engage a larger area of available bone and create a wider support base — eliminating the need for bone grafting in most patients. This allows the force of chewing to be distributed evenly across all four implants.

The result is a fully fixed prosthesis supported by four implants — as opposed to replacing each tooth individually with 10–14 implants. It is a more efficient, more predictable, and more cost-effective solution for full-arch replacement.

Full arch of teeth anchored on just 4 implants
Fixed, permanent — no removal, no adhesives, no slipping
Same-day teeth possible for qualifying patients
Preserves jawbone and prevents facial collapse
Fraction of the cost of replacing each tooth individually
Immediate improvement in chewing, speech, and confidence
15–20 year longevity with proper care

The Process

From First Scan to
Permanent Smile

01

CBCT 3D Assessment

Cone Beam CT scan maps your bone in 3D — revealing exact bone volume, nerve positions, and the precise implant positions needed. No guesswork.

02

Digital Treatment Plan

Using the CBCT data, we digitally plan implant positions, angles, and the final prosthesis design. You see your result before surgery begins.

03

Implant Surgery

Four titanium implants are placed strategically — two upright at the front, two angled at the back to maximise bone contact. Done under local anaesthesia. Most patients are surprised by how comfortable it is.

04

Immediate Teeth (Same Day)

For qualifying patients, a fixed temporary prosthesis is attached the same day. You leave with teeth. The permanent prosthesis is fabricated over the following months.

05

Final Prosthesis

After full osseointegration (3–6 months), the final high-strength zirconia prosthesis is fitted — a complete, beautiful, permanent set of teeth.

Common Questions

Am I a candidate for All-on-4?

Most adults who are missing all or most teeth in an arch qualify. Even patients with significant bone loss often qualify because the angled implants engage areas of denser bone. A CBCT 3D scan is required for accurate assessment.

Is the surgery painful?

The procedure is performed under local anaesthesia, often with IV sedation for anxious patients. Most patients report far less discomfort than anticipated. Post-operative soreness is managed with prescribed medications and resolves within 3–5 days.

How long do All-on-4 implants last?

With proper oral hygiene, the implants themselves can last a lifetime. The prosthetic teeth (the visible part) typically last 15–20 years before potential replacement due to wear. Many patients never need their prosthesis replaced.

What do I eat while healing?

For the first 2–3 months while the implants are integrating, a soft food diet is recommended. After full osseointegration and fitting of the final prosthesis, you can eat virtually anything — including foods that denture wearers cannot manage.

What is the difference between All-on-4 and All-on-6?

All-on-6 uses two additional implants for greater load distribution and is sometimes preferred in the upper jaw where bone density is typically lower. We assess each patient's bone architecture and recommend the optimal number for long-term success.
